succeed in their homes, schools, and communities.

• Improve functioning, coping, self-management, healthy relationships, and coordination of care.

• Engage youth and parents or guardians in individualized service planning as appropriate.

• Promote safe continuity of care and appropriate community referrals; evaluate service outcomes and access.

Population served and admission approach

regulation challenges, or difficulties developing positive peer relationships. A diagnosis or presenting concern alone does

not establish eligibility for every service.

Admission and service intensity depend on individualized assessment, scope of certification, personnel qualifications,

medical necessity when applicable, informed consent, safety and referral appropriateness.

Certified services provided by GH&W

General Services

GH&W provides general behavioral health assessment activities conducted by qualified professionals to identify presenting

concerns, safety risks, strengths, treatment needs, diagnosis as applicable, and appropriate next steps. Assessment

informs person-centered treatment planning and referrals.

Population and goals: Youth and families seeking evaluation or treatment planning for emotional or behavioral health

concerns. Goals include identifying needs and risks, selecting appropriate services, developing individualized plans, and

linking to outside resources when needed.

Methods: In-person and/or approved telehealth

Community Psychiatric Supportive Treatment (CPST)

Description and scope: CPST consists of individualized and group, measurable community psychiatric support that

addresses assessed mental health needs and helps individuals function successfully in the community. Depending on need,

authorized activities may include ongoing assessment, daily living skill development, service-plan coordination, access to

natural and formal supports, symptom monitoring, crisis coordination, outreach, advocacy, and individualized education

linked to assessed needs.

Population and goals: Youth and families needing coordinated mental health support to improve functioning at home,

school, or in the community. Goals include improved independent functioning, linkage and follow-through with needed

resources, strengthened natural support, and reduced barriers caused by psychiatric symptoms.

Delivery and staffing: Services may occur at GH&W, in appropriate community settings, and through permitted

communication methods, provided qualified staff deliver and document them under required oversight. A designated GH&W

CPST staff member is responsible for case coordination. GH&W retains service and billing accountability.

Therapeutic Behavioral Services (TBS)

Description and scope: TBS provides individualized and group behavioral support and care coordination for people whose

assessed mental health needs interfere with daily functioning. Activities may include consultation for treatment planning,

referral and linkage, interventions using evidence-based techniques, identifying strategies and treatment options, restoration

of social skills and functioning, and crisis prevention or amelioration.

Population and goals: Youth experiencing symptoms or behavioral challenges affecting self-regulation, coping, peer

communication, problem-solving, and functioning at home, school, and in the community. Goals are individualized and

measurable.

Delivery and staffing: Individual and group interventions when appropriate to the person’s plan, furnished by staff meeting

applicable qualifications.

Clinical treatment planning and safeguards

GH&W develops an individualized service plan based on assessment and documented need, with participation from the

person who served and, as appropriate, the parent, legal guardian, or other authorized support. Staff identifies measurable

goals, selected services, interventions, frequency/intensity as appropriate, progress measures, referrals, and anticipated

review points. Clinical records support the service delivered, staff qualifications, time and location, participant response and

progress, according to current payer and regulatory rules.

Growth Operations collaboration and integrative programs

Growth Operations (Growth Ops) is a separately registered nonprofit organization that develops youth-enrichment and

community-engagement opportunities. The organizations share youth-development goals but maintain separate legal

identities, service responsibilities, personnel oversight, and financial accountability.

GH&W recognizes the potential benefits of education, physical wellness, constructive peer relationships, and service

opportunities alongside individualized and group clinical treatment.

The Four Mediums of Growth

Education and life skills

Growth Ops promotes learning and self-efficacy through Gardening, After-School Study Tables, Literacy Programming,

Social-Emotional Learning Workshops, STEM, and athletic support. These activities support academic engagement,

decision-making, emotional awareness, and science, technology, engineering, and math.

Community service and leadership

The Growth Ops Service Platform introduces opportunities such as volunteering with local partners, neighborhood projects,

and awareness initiatives. Youth may practice collaboration, empathy, accountability, and leadership.

Peer socialization and community engagement

Growth Ops creates opportunities for positive peer relationships, communication, cooperative experiences, and social

confidence. Its community outreach events may bring together youth from schools and neighborhoods across the Toledo

area.

Physical activity, wellness, and brain fitness

Growth Ops may offer supervised fitness and movement, outdoor activities, nutrition education, mindfulness exercises, and

games that encourage concentration, memory, and problem-solving. Activities should be appropriate to youth age, needs,

and safety considerations.
